I’m reminded of my brilliant friend Andy, who had a number of rants that were a bit like stand-up comedy routines. One of them started with “Make me the captain of the Enterprise, and the FIRST thing I do, the VERY FIRST THING, is rip out that holodeck and leave it floating in space somewhere.”
His premise being, in what universe is an ENTERTAINMENT DEVICE allowed to threaten the lives of the crew of a military vessel?

More than once per reincarnation. The holodecks were always malfunctioning!
